School Registration Forms and Information

Whether your student is enrolling in a brick and mortar school or in one of the Virtual programs, you and your student will need to fill out the following:


2019-2020 School Registration Packet


The following are required for enrollment:

  • Birth Certificate
  • Two proofs of residency (with at least one in the parent/legal guardian's name); utility bill or hookup order , lease or rental agreement, mortgage agreement, income tax form or homestead exemption
  • Immunization records
  • A copy of a physical The physical MUST have been given within a year (12 months) prior to the DAY the student is enrolled .

Section 1002.20(24), Florida Statutes, requires each middle school and high school student or the student’s parent be provided a two-page summary of the Department of Economic Opportunity’s economic security report of employment and earning outcomes. This must be done annually prior to registration and electronic access to the full report must also accompany the summary. The following links are to the summary and full economic security report.
